Health Home is not a place. It is a voluntary service that Heartlinks provides to Molina Healthcare insurees of all ages, working with you to manage your healthcare and life journey. You can think of Heartlinks as a bridge—or link—between you and the care you need and want.

What is Health Home?

Heartlinks can be your local Care Coordinator helping you navigate your health care and social service needs. You are not our patient, you are our client and we work to help you meet your goals.

  • Focused on your needs and desires for daily living

  • Helping you improve your overall health and wellbeing

  • Convenient face-to-face appointments in a place where you are comfortable (your home, a Heartlinks’ office, or a public space in your community)

Heartlinks can help you:

  • Create a Health Action Plan

  • Identify and create holistic goals for your medical, emotional, and spiritual care

  • Connect with the right people and places to meet your goals

  • Communicate and make appointments with multiple doctors, therapists, and other providers

  • Find helpful services such as transportation, food, housing or medical supplies

  • Avoid unnecessary hospitalizations or treatment

  • Communicate with your family, caregivers and healthcare providers

  • Assist your transition to home after visiting a hospital or care facility

Health Home at No Cost to You

Health Home is a program through the Washington State Health Care Authority, the Centers for Medicare and Medicaid Services (CMS), and The Department of Social and Health Services (DSHS).

Health Home is available to residents of the State of Washington. The cost is covered by your insurance provider.
