Care Coordination is a voluntary service that Heartlinks provides to Molina Healthcare insurees of all ages, working with you to manage your healthcare and life journey. You can think of Heartlinks as a bridge—or link—between you and the care you need and want.
What is Care Coordination?
Heartlinks can be your local Care Coordinator helping you navigate your health care and social service needs. You are not our patient, you are our client and we work to help you meet your goals.

Connect with the right people and places to meet your goals

Communicate and make appointments with multiple doctors, therapists, and other providers

Find helpful services such as transportation, food, housing or medical supplies

Avoid unnecessary hospitalizations or treatment

Communicate with your family, caregivers and healthcare providers

Assist your transition to home after visiting a hospital or care facility

Care Coordination at No Cost to You
Care Coordination is a program through the Washington State Health Care Authority, the Centers for Medicare and Medicaid Services (CMS), and The Department of Social and Health Services (DSHS).
Care Coordination is available to residents of the State of Washington. The cost is covered by your insurance provider.

Make a Care Coordination Referral
It can be difficult for people to access the services and resources they need to thrive. Heartlinks partners with the Greater Health Now Community Hub to connect individuals and families with community-based care coordination services.
Referrals can be made by individuals, family members, caregivers, or healthcare providers.

How It Works

An individual is referred (or self-refers) to the Community Hub.

Greater Health Now connects them with a trusted Community Health Worker.

The Community Health Worker reaches out to discuss the individual's needs and goals.

The Community Health Worker helps connect them with community resources and follows up to ensure they receive the support they need.
