Through your generous support, Heartlinks serves more than 1,400 individuals and their families annually. We have seen a 23% increase in the need for hospice, palliative care, and grief support, and we anticipate helping even more families in 2025.

As you consider giving to the many organizations dear to your heart this Giving Tuesday, we want to share how your past donations have impacted the lives of those we serve at Heartlinks through hospice care, pediatric palliative care and our Adult Family Home.

More Family Time for Mae – Hospice  Care

At 70, Mae faced the challenges of ALS, a terminal neurological disease that progressively stole her mobility and independence. A dedicated worker for over 25 years at a processing warehouse, Mae now lives with her daughter and two young grandchildren, relying on an assistive technology tablet to communicate. As her condition has advanced, she now requires specialized feeding supplies, respiratory support through a VOCSN system, and a PureWick system to maintain her comfort and dignity.

While many hospices turn away ALS patients due to high costs, Heartlinks said yes, providing over $1,200 worth of advanced equipment and care to meet Mae’s needs.

Heartlinks’ team went beyond traditional hospice services, offering essential tools, personalized care, and emotional support for Mae and her family. Nurses trained her daughter to confidently manage Mae’s equipment, while social workers and spiritual counselors brought connection and comfort into their home.

Mae typed, “Thank you for helping me stay with my family,” reflecting the profound impact of this compassionate care.

Mobility for Janae – Pediatric Palliative Care

At just five years old, Janae is already a warrior. Born with a genetic disorder that limits her mobility, Janae requires constant care. She’s been part of Heartlinks’ pediatric palliative program for two years, receiving the support and compassion she needs to thrive. But Janae faces obstacles beyond mobility.

Her grandmother, who cares for and raises Janae, struggled to transport her grandchild due to the limitations of a traditional, heavy wheelchair that also lacked the ability to secure Janae safely since she is unable to sit up on her own.

Upon learning this, Heartlinks reached out to the community for help, and friends like you stepped up!

Thanks to the extraordinary generosity of donors, a life-changing activity stroller—valued at $1,800—was made possible. This stroller isn’t just a piece of equipment; it’s a lifeline. It’s light enough for Grandma to manage and perfectly designed to support Janae’s needs, allowing her to comfortably join her family on outings, connect with the world outside her home, and embrace moments that once felt impossible.

Janae’s little brother has even taken on the role of “designated driver,” pushing her with enthusiasm and joy. For him, it’s not just a duty but an opportunity to share adventures and memories with his sister.

Transformation for Linda- Adult Family Home

Linda (name changed for privacy) recently became part of our Adult Family Home family. She has Alzheimer’s and was unable to communicate when she moved in. At the time, she was receiving hospice care due to her severe wounds, pressure sores and consistent weight loss.

When Heartlinks opened the Adult Family Home on October 1, we welcomed Linda with open arms and a commitment to provide the best care possible. In just two months, her progress has been nothing short of miraculous. Her wounds have healed, her strength is returning, and she has been discharged from hospice care due to her remarkable improvement.

Thanks to personalized care and the loving support of her family, who spend several hours a day with Mary, she has experienced a transformation. For the first time in years, she can get out of bed. Even more inspiring, she has regained the ability to communicate through nods and gestures, finding her unique way to connect with caregivers and loved ones.
