What is an adult family home?

The Heartlinks Adult Family Home is just that, a residential home licensed for 24/7 senior care in a small, family-oriented environment. Different than an assisted living facility, the Heartlinks AFH has just four bedrooms that can accommodate six people who will share a family room, living room, kitchen, bathroom and outdoor garden area. Laundry, cleaning and meals will be provided as well as assistance with personal care by a staff of at least three people you will get to know on a personal level.

How is an adult family home different from an assisted living facility?

An adult family home in Washington state is licensed to provide housing and care for up to six adults in a regular house located in a residential neighborhood. AFHs that have been licensed for two years may apply to increase their capacity to eight residents if they meet certain criteria. An assisted living facility provides housing and care for seven or more people in a home or facility in a residential neighborhood. Both provide a room and meals, laundry, supervision, assistance with activities of daily living and personal care. Adult family homes offer more personalized care as the staff to resident ratio is smaller.

How is Heartlinks different from other adult family homes?

Heartlinks is the only Adult Family Home in Sunnyside, Washington, and one of very few in the lower Valley. We hope to serve the residents of the lower valley whose family can remain close to home.

Heartlinks’ 45 years of expert guidance, care, compassion and support for aging adults in the Lower Valley, Yakima and Tri-Cities allows us to provide care through the end of life for anyone living in the Adult Family Home. If needed, we can provide hospice, palliative and respite care.

As a nonprofit organization and through the generous giving of our supporters, Heartlinks can provide services at the lowest price possible and offer charity if needed.

Although the staff working within the Adult Family Home is modest, the local Heartlinks team of medical caregivers and administrative staff is 61 people strong. All Heartlinks employees live and work in Benton and Yakima counties.

How many residents live here?

The Heartlinks Adult Family Home has four bedrooms that can house up to six residents.

  2. One Suite designed for a married couple
  3. One two-person room designed for roommates
Is there a minimum or maximum length of stay?

Depending on availability, our respite care length of stay is based on the individual’s care needs, but would likely be less than 7 days. For long-term residents, there is no maximum length of stay.

Who will provide 24/7 care and what training do they have?

Day and night, our home will be staffed by Certified Nursing Assistants and Home Care Aids under the supervision of a Registered Nurse and Clinical Director. All staff are trained in long-term caregiving, safety, first aid, CPR, food handling and safety, dementia and mental health, hospice and end-of-life care. Residents can continue to see their preferred doctor.

How much do room, board, and services cost?

Prices are based on the level of care and assistance required by the resident. Prior to admission, a nurse will meet with the family to assess the needs, concerns, and special accommodations required. Our services may be covered under Medicaid, private insurance plans, HMOs, or veteran’s benefits.

Charitable care may be provided on a case-by-case basis.

What insurances do you accept?

Our services may be covered under Medicaid, private insurance plans, HMOs or veteran’s benefits. Medicare does not cover room and board but may cover hospice or palliative care provided at the Heartlinks Adult Family Home.

What amenities are included?

Meal Preparation

  • 3 nutritious meals daily, plus snacks and beverages
  • Monitoring of diet if prescribed (i.e., low sugar, no salt, etc.)
  • Assistance with meals as needed

Housekeeping and Laundry Service

  • Basic housekeeping
  • Regular laundry service
  • Linens, sheets, pillows and blankets included

Furniture And Basic Supplies

  • Bedroom furniture (bed, side table, chair and dresser or closet)
  • Living and social areas with a TV
  • Toiletries and hygiene supplies

Medication Management

  • Dispensing of medicines per doctor's orders
  • Medication inventories are stored and locked
  • Ordering prescriptions and refills

Personal Care

  • Escort to and from meals and activities as needed
  • Assistance with grooming, bathing, and dressing as needed
  • Observe and report any changes in appetite or behavior
  • Robust activity calendar tailored to residents’ interests

Clinical Care

  • Individualized and specialized care plans
  • 24/7-hour licensed care staff for supervision, safety, and support
  • Choice of the physician or Heartlinks’ physician support
  • Outside services of hospice care, palliative care, home health, physical therapy, speech therapy, etc.
  • Wound care and continence supplies
  • Durable medical equipment
Are pets allowed?

At this time, Heartlinks cannot guarantee the safety and care of pets within the Adult Family Home and therefore does not allow them within the home. Our yard is not fenced, and the safety and care of our residents is our top priority.

Is transportation to activities outside the home provided?

Transportation to and from the Adult Family Home is not currently provided, but the staff at Heartlinks can help coordinate a paid transportation service for the residents as needed. Residents are allowed to leave the home with an approved family member or friend.

What languages do the Adult Family Home staff speak?

English and Spanish are both spoken by the Heartlinks staff.
