At the Heartlinks Adult Family Home, you are part of the family.

Our four-bedroom home in Sunnyside provides 24/7 licensed care in a loving, group setting that offers the privacy of individual bedrooms.

The Heartlinks Adult Family Home is located at 3920 Outlook Road in Sunnyside. The large parking lot and driveway make visiting family easy to do. The entire home is wheelchair accessible.

Welcome to the Heartlinks Adult Family Home. This is the view from the entrance. The laundry facilities are through the barn door.

Single rooms are large enough for a full-size bed, chair and nightstand. All rooms will either be furnished with a wardrobe or a closet.

Residents may share this two-person, semiprivate room and decorate it as they please. Beds, nightstands and a wardrobe or closet will be provided.

The large bathroom is equipped with grab bars, a roll-in shower and a roll-under sink.

The family activity room has enough space to sit and enjoy a conversation. The table is a great space to complete puzzles, crafts and other activities residents can enjoy together.

This space will include a television as well as individual spaces for reading or enjoying a conversation.

Residents will share three nutritious meals a day here. This space can also be used for families to celebrate birthdays, anniversaries or other occasions where treats are served.

The kitchen is fully stocked with everything to make and enjoy home-cooked meals. From the kitchen, staff can easily see and hear residents in the family activity room, dining room and living room.

What is an Adult Family Home?

The Heartlinks Adult Family Home is just that, a residential home remodeled and licensed for 24/7 senior care in a small, family-oriented environment. Different than an assisted living facility, the Heartlinks AFH has just four bedrooms that can accommodate six people who will share a family room, kitchen, bathroom and outdoor garden area. Laundry, cleaning and meals will be provided as well as assistance with personal care by a staff of at least three people you will get to know at a personal level.

Who is eligible to live in the Heartlinks Adult Family Home?

How is an Adult Family Home different from an assisted living facility?

An adult family home in Washington state is licensed to provide housing and care for up to six adults in a regular house located in a residential neighborhood. AFHs that have been licensed for two years may apply to increase their capacity to eight residents if they meet certain criteria. An assisted living facility provides housing and care for seven or more people in a home or facility in a residential neighborhood. Both provide a room and meals, laundry, supervision, assistance with activities of daily living and personal care. Adult family homes offer more personalized care as the staff to resident ratio is smaller.

How is Heartlinks different than other Adult Family Homes?

Heartlinks is the only Adult Family Home in Sunnyside, Washington, and one of very few in the lower Valley. We hope to serve the residents of the lower valley whose family can remain close to home.

Heartlinks’ 45 years of expert guidance, care, compassion and support for aging adults in the Lower Valley, Yakima and Tri-Cities allows us to provide care through the end of life for anyone living in the Adult Family Home. If needed, we can provide hospice, palliative and respite care.

As a nonprofit organization and through the generous giving of our supporters, Heartlinks can provide services at the lowest price possible and offer charity if needed.

Although the staff working within the Adult Family Home is modest, the local Heartlinks team of medical caregivers and administrative staff is 61 people strong. All Heartlinks employees live and work in Benton and Yakima counties.

How much do room, board and services cost?

Prices are based on the level of care and assistance required by the resident. Prior to admission, a nurse will meet with the family to assess the needs, concerns, and special accommodations required. Our services may be covered under Medicaid, private insurance plans, HMOs, or veteran’s benefits.

Charitable care may be provided on a case-by-case basis.
