If, in your best judgment, your patient has a prognosis of six months or fewer, or if the illness has run its typical course, your patient is eligible for Hospice. At regularly prescribed intervals during your patient’s time on Hospice Care, we will reassess the medical appropriateness.

If your patient’s prognosis is more than six months, or if your patient wishes to pursue curative treatment, please consider referring to our Palliative Care program.
