Heartlinks Hospice & Palliative Care is a nonprofit Hospice, and the communities #1 choice in Hospice and Palliative Care. Referring your patient to Heartlinks gives them and their family a sense of reassurance and the kind of support they need at the end-of-life.
Heartlinks helps the patient and their loved one make the very most out of every day in their final months of life. Our interdisciplinary team will care for them wherever they call “home”, and help them have comfort, peace of mind, and dignity during life’s final stages.
If, in your best judgment, your patient has a prognosis of six months or fewer, or if the illness has run its typical course, your patient is eligible for Hospice. At regularly prescribed intervals during your patient’s time on Hospice Care, we will reassess the medical appropriateness.
If your patient’s prognosis is more than six months, or if your patient wishes to pursue curative treatment, please consider referring to our Palliative Care program.
If you wish to consult about a prospective Hospice or Palliative Care patient,
please call our office and ask to speak to one of our Referral / Intake Nurses:
Billie Brown (Yakima County), and Jennifer Miner (Benton County) at 509-837-1676.
