Area of Greatest Need: Heartlinks Hospice is dedicated to enhancing the quality of life by providing loving care to the dying and support to their families. Heartlinks Hospice provides medication, medical supplies, equipment, and other unreimbursed patient care items such as nutritional supplements and special needs.
Heartlinks Fund: Heartlinks Hospice does not turn away patients based on their inability to pay. It is our mission to give our neighbors the best possible care whether they can afford it or not. This fund allows us to serve these well-deserving patients.
Adult Palliative Care Fund: Palliative Care is a specialized medical care for people with serious life-threatening medically complex disease processes. It is focused on providing relief from the symptoms, pain, and stress of the disease. The goal is to improve quality of life for both the patient and family.
Pediatric Palliative Care Fund: Nurse case management support for children ages birth to 20 years with complex medical conditions with progression of symptoms and loss of function or physical decline. The supportive team consists of a Registered Nurse and a Social Worker.
